Assessment

On enrollment to Van Dyk Memory Center, Dementia certified staff conduct a complete assessment. Our staff listen to our members and family expectations to design an individualized activity program. We always encourage member input, offering each member decision making while our trained staff balance the risks and responsibilities. The Memory Care Center’s staff follow the latest studies of activities found successful by accredited organizations and researchers studying dementia and Alzheimer’s disease and incorporate the activity into the Center’s plans.

At Van Dyk Memory Center we want our families to have available resources for leisure activity and care solutions while at home. Our goal is to enable our members in achieving and maintaining the highest level of capabilities for as long as possible. Access to “at home” activities to maintain self-esteem and independence for our members are tailored by our staff offering our families a personalized activity planning guide to assist in the hours away from the Memory Care Center.

While a member of the Memory Care Center monthly Wellness checks consisting of weights and blood pressure reading assist families in monitoring their family member, providing information that may require a doctor appointment.

Communication

Communication is paramount for the Van Dyk Family and Staff. Unique to Van Dyk Memory Care Center is our ability to connect to our families through a mobile application in real-time communication. This customized free application is available with secure log-in features on a cell phone. Texts, emails, photos, and videos can enhance the enjoyment of knowing how a member’s day is progressing. Using the customized application families can receive center announcements, reminders and highlighted events happening at the center. A wonderful feature providing each family and caregiver another measure for true peace of mind.

Dining

Daily at the Memory Center nutritious meals and snacks are prepared fresh by the Van Dyk chefs and served to our members. A morning continental breakfast, three course lunch and afternoon refreshments offer the member a positive social opportunity and a balanced nutritional planned menu approved by a Registered Dietician. Special holiday menus, birthday celebrations along with live cooking demonstrations and member participation will energize the culinary experience.

Van Dyk Family Network

In our continuum of care there are physicians, practitioners, Registered Dieticians and Social Workers who see patients in our assisted living, post – acute and skilled nursing services. We can also recommend a Certified Personal Trainer, Pharmacies and a Medical supply company. The listing is available to all our Memory Care members. We consider these professionals a component of the Van Dyk Family Network of Medical Providers.
