Each team member of Van Dyk Memory Care Center is certified in Alzheimer’s Care. Our team meets monthly for review of basic and advanced dementia care techniques. Van Dyk Memory Care Center continually follows all breakthroughs and developments with the disease and effects of aging and adapts evidence based best practices for Alzheimer’s.
Alexa Green
Director, Van Dyk Memory Care Center
Alexa Green has been a proud member of the Van Dyk family for many years, showcasing her extensive background in sales, marketing, community relations and activities. As Director of the Van Dyk Memory Care Center, Alexa oversees all operations, including program development, activities, staffing, marketing, and business development. She previously served as Director of Sales and Marketing at Van Dyk Park Place, managing all admissions, tours, advertising and communications for Van Dyk’s award-winning assisted living community. Alexa’s first leadership role at Van Dyk Park Place came as Community Lifestyle Director, a role in which she served for several years — developing innovative programs and activities for residents in assisted living and memory care, Her commitment to exceeding expectations and helping families find solutions has made Alexa a true leader in this profession. She is dedicated to cultivating strong relationships with seniors, families and strategic partners. Among her many leadership roles, Alexa has represented the Van Dyk family as a member of the Dementia Care Provider Roundtable, a national board of healthcare leaders serving the Alzheimer’s Association. She originally joined the Van Dyk family when she was a freshman at DePaul Catholic High School, volunteering during summer months and holiday seasons. She served as an intern at Van Dyk Park Place during her junior year of college and later accepted a full-time position in the activities department. Alexa earned her bachelor’s degree in Health Care Administration with a minor in Business Administration (and a concentration in Long-Term Care) at the University of Scranton. Alexa’s story at Van Dyk is truly remarkable, starting as a volunteer and now serving on Van Dyk’s leadership team. Her energy, warmth and compassion are well known to families and health care professionals alike.
Pam Tokarczuk, LNHA, CALA, CDP
Family Liaison, Van Dyk Memory Care Center
Pam Tokarczuk has been one of the leading memory care experts in New Jersey for over 20 years. She works closely with families whose loved ones require memory care in our LightHouse and Reflections neighborhoods at Van Dyk Park Place, as well as our adult day care program. In her decorated career, Pam has served as Executive Director of Van Dyk Park Place, Executive Director of Van Dyk Memory Care Center, Administrator of Van Dyk Manor in Ridgewood (recognized for five consecutive years by US News and World Report as a 5-star post-acute rehabilitation and skilled nursing facility), and Executive Director of Van Dyk at Bald Eagle Commons Assisted Living and Senior Apartments in West Milford, NJ. Pam has managed the quality and consistency of care services and operations, maintaining quality standards and compliance on behalf of Van Dyk Health Care. As a Certified Dementia Practitioner, Pam is known for her compassionate care and resident advocacy. She serves on the NJ LTC Culture Change Committee, HCANJ Best Practice Committee and Convention presenter for “Changing the Culture of Dementia Care to Reduce the Inappropriate Use of Antipsychotic Medication”. She graduated from Fairleigh Dickinson University with both bachelors and master’s degrees, and has since earned nursing home administrator license, assisted living license for the New Jersey Department of Health and Senior Services and is certified with the National Council of Dementia Practitioners.
